We advocate for our young people to become active members of their community
We understand transition is a gradual process, therefore, we work with Children's Services by preparing, planning, and moving into Adult Services. This will enable us to bridge the gap through their transitional phase in line with the NICE guideline Transition from children to adults’ services for young people using health or social care services and the Care Act (2014). The transitional development of our young people is monitored through robust care planning systems and monitoring tools and our approach is collaborative, flexible and inclusive.
Individualised care packages and treatment plans (developed by health care professionals) will work in conjunction with the young person and their support networks. We aim to maximize our plan using least restrictive practices. The combination of these plans focusses on therapeutic methods and de- escalation techniques.
It is important to us, to include the young person in the development of their care package, ensuring that it is flexible throughout their journey. The care package should be adaptable according to the needs of the person throughout their time with us.
Johnson's House is a Residential Care Home Transitional Service, supporting young people from the ages of 16 - 24 years old, providing 24-hour care and support. We support our young people by delivering a holistic care and support package. We aim to provide a safe, stable, and consistent living environment.
Our service focuses on the transitional stages young people experience, supporting young people with mental health, challenging behaviour, and other complex needs.
As a care provider we believe in actively breaking down barriers wherever possible. As part of our comprehensive approach, we promote and actively encourage young people in gaining access to local facilities, social, cultural, and health opportunities. Whilst also supporting them with public transportation, educational, training and employment options, as well as facilitating recreational activities and events. We advocate for our young people to help them become active members of their communities.
We specialise in and have extensive expertise caring for young people with mental health issues and other who require round-the-clock care owing to multiple complex needs and/or learning and behavioural difficulties.
Our knowledgeable and experienced staff members collaborate with local and regional health and social care teams to provide top-quality, personalised care in a safe and comfortable setting.
